It was all just too embarrassing, particularly because we all knew there was little medical science could do to solve the problems. So all this embarrassment was put into a box with the label “impotence” stuck on to the outside and it was put away on a high shelf where no one could see it by accident. Except, of course, this didn’t stop men from finding they did have problems and, if the news leaked out, these men found themselves the butt of jokes and, worse, mockery. We’ve never been very kind to each other when we all fear the same problems. So, working quietly in labs around the world, teams of researchers have been trying to solve a range of different sexual problems. For example, there are men who can achieve a hard erection but cannot then reach an orgasm. This is deeply frustrating for the man and, if the woman is interested in pregnancy, somewhat annoying. Less obvious but no less annoying when it becomes apparent are those men who do have an orgasm but do not ejaculate, or only produce very low volumes of sperm, or do not produce active sperm. Fertility is something that can only be proved by scientific test and it’s not unknown for women to ask for sperm samples for testing before agreeing to marriage. All this before we get to the men who don’t produce an erection.
Sex is a complicated area of medicine because it mixes physical ability into social situations. If the body fails, it may be because the mind is not involved. It’s a fact no man produces an erection unless he finds the situation sexually stimulating. So if he’s not genuinely attracted to the woman, there will be no erection. That’s why erectile dysfunction and depression are always linked. When the man grows too sad, he fails to find any situation sexually simulating. What about the other sexual problems?
Sunday, September 25th, 2011One of the fascinating things we can do as human beings is put information in little boxes and pretend that’s all there is to know. It’s like when you’re at High School and, for the next test, all you need to know is [insert information]. Once you pass on to the next level, you can often forget whatever you learnt for all those tests. The lid has closed on the box and, unless there’s a crisis, you need never try to remember it again. Well, it’s like the world decided it wanted a smaller box for all the information on sexual problems. In the bad old days, we used to talk about impotence and that included quite a lot of different stuff. Now we only seem to talk about erectile dysfunction and there’s not much to say about that. All the other stuff about an inability to have an orgasm or to ejaculate, low levels of sperm or infertility has disappeared. It’s like we’re not supposed to think about anything like that now we have the erectile dysfunction dugs.
Yet, if you ask the doctors who specialize in this area, they’ll tell you that about two-thirds of the men with severe erectile dysfunction also find it difficult to reach an orgasm, and about 60% find it difficult to ejaculate. To understand what’s happening, we have to understand how drugs are marketed. When a pharmaceutical manufacturer comes to market with something new, it’s vitally important to establish a connection between the brand name and the specific condition it treats. To make this drug stand out, the other diseases and disorders in the same part of the body have to be minimized. The important problem has been solved. The marketers prefer you not to ask about the others.
So, because of this manipulation of the general discussion about sexual problems, the only one we focus on is erectile dysfunction and we do that because we have an effective treatment. Where does that leave us with all the men who cannot ejaculate or no longer have an orgasm? Well, when we do go to the doctor, we fail to report these other problems, and the doctors fail to ask us because there’s no treatment currently available. Yes, sadly, the erectile dysfunction drugs only treat the named condition. They have no effect on the other problems.
When you go back to the original statistics, two-thirds of men with severe erectile dysfunction have no remedy for the secondary problems. They could therefore find hard erections return, but they are still not able to have full sexual intercourse. For older men, children are no longer on the agenda, so their inability to produce children will be less important. Sex for diabetic men
Saturday, September 17th, 2011Diabetes is an increasingly common problem as more people carry too much weight around from ever younger ages. Untreated there are two major problems that follow. There’s a reduction in the flow of blood around the body and there’s increasing damage to the nerve endings. As a result, about three-quarters of all diabetic men suffer some degree of erectile dysfunction. The national health agencies confirm a steady statistical trend. All men begin to lose some degree of sexual function as they age but sexual dysfunction shows up in diabetic men ten to fifteen years before healthy men. As a general warning, if the diabetes is not treated and there’s damage to the nerve endings, sexual function does not return. Nerve endings do not regenerate. It’s therefore always in your interests to have the tests for diabetes if you are overweight and then begin to suffer erectile dysfunction. Although it takes several years for the nerve endings to be damaged, it’s foolish to take any risks.
A recent study in Australia has offered the encouraging news that, if you lose 5% or more of your body weight, this is likely to restore sexual function and can also help with the bladder problems likely to afflict diabetic men. The study divided the participants into two groups who tried different diets. The results show the nature of the diet to be irrelevant. As soon as the body weight was reduced, all the participants reported improved sexual performance. The previous studies have examined the use of surgical procedures to reduce weight. Not unnaturally, with a band in place, more weight is lost. This is the first study to show that an ordinary diet producing not less than 5% weight loss is equally effective.
This is a dramatic result because it suggests major invasive surgery may not always be necessary. The decision should depend on a detailed consideration of all the health problems affecting the overweight man. There’s just one problem. The study only involved thirty-one men. This is nowhere near enough participants to produce scaleable results. So, unless and until there’s a properly designed trial involving at least 500 men, we cannot rely on these results.
